Well, checking a fast facts site, I expect to see facts fastly.

One fact I see, per this site, is not 9M but 26M (many of whom don't know it. Which is rather like the Vit-D deficiency numbers OPK has put out in numerous presentations. You have it, but you don't know it.)

And, unless I am way too dense to be discussing anything but a good game of checkers, this only enforces my former assertion that OPK is lowballing the $12B market stuff. Or these guys at the National Kidney Foundation are just painting a dire picture.

Now, as somebody pointed out, Bishop said Rayaldee might be able to "prevent" SHTP. And how do you do that? Standard answer is early detection. But what is the point of early detection if the docs have nothing to offer to prevent it? Why even bother detecting something you have no answer to? If causality has anything to do with diet and exercise, then they know why not. It's the same reason (among others) all weight loss drugs eventually fail. People are going to do what they do.

Had a good friend, was a clerk for a Federal judge (no no dummy), had lung cancer and continued to smoke cigarettes till the day she died. Point being, if prevention or remediation of a problem is dependent largely upon behavior modification, well.......good luck with that.

But I digress. Looks like the market is even bigger than I thought after thinking it was bigger than what OPK was telling us. Way bigger.

Approximately 69% of CKD patients are enrolled in Medicare Part D, including both the stand-alone and Medicare Advantage plans. This is slightly higher than the Part D enrollment in the general Medicare population (66%).
CKD continues to be a major cause of lost productivity, physician visits and hospitalizations among men and women. The average annual number of physician visits for an individual with diagnosed kidney disease is 10.28 -- second only to cancer.

Medicare spends $31 billion annually to treat people with kidney failure. In 2013, total Medicare expenditures for all stages of kidney disease was over $99 billion (not including prescription medications). The majority of these expenses, about $68 billion, were spent caring for those with chronic kidney disease.

Federal law currently prohibits the government from negotiating with the pharmaceutical industry to lower the cost of drugs purchased under Medicare Part D. Instead, PDPs (Medicare prescription drug plan or Insurance Provider) are required to negotiate directly with pharmaceutical manufacturers to obtain rebates and other discounts on drugs. So OPKO will be negotiating with Part D plan sponsors.

The ten largest Part D plan sponsors in 2014--in order of market share--are UnitedHealth Group, Humana, CVS Caremark, Express Scripts, Aetna, Cigna, WellCare Health Plans, Kaiser Permanente, WellPoint and Envision.
These 10 companies account for more than three-fourths of all enrollees, with UnitedHealth alone accounting for more than one in five Part D enrollees, including 22 percent of prescription drug plans (PDP) enrollees and 21 percent of Medicare Advantage prescription drug plans (MA-PD).
