Page 11 of the manual reads:

"There are several exceptions to the requirement that an item or service be reasonable and
necessary for diagnosis or treatment of illness or injury. The exceptions appear in the full
text of §1862(a)(1) and include but are not limited to:"

And the list includes "Prostate cancer screening tests are covered if within frequency limits;"

This would seem to be a directive to cover 4kscore, and this brings up the question of the title of the Palmetto LCD which uses the term "Assay" and not "Test" as is reflected in the Novitas LCD.

Also icemandios, the DL numbers are different: DL36763: 4Kscore Assay-Palmetto GBA and DL36736:4KScore Prostate Cancer Test-Novitas Solutions, Inc. (I also can't access the Novitas website for some reason, maybe it's because I'm in Canada?
